Insurance companies and payment are
subject to terms of contracts between the insured and that company. We
will do our best to verify all insurance information and we will bill for
our patients.
However, ultimate payment for services rendered in this office is the
responsibility of the patient and payments are requested at the time of
service unless other arrangements have been made.
